CBA PROGRAM FEE STRUCTURE

Sunshine Enterprises is offering registration at a special price of $99. Note that Financial Aid is available for those that qualify.

If you believe paying the CBA Program Fee will be a financial hardship because of your current income status, choose Yes.

If you selected YES above you can apply for financial assistance. To be considered for financial assistance, you MUST provide the following income documentation via email to recruitment@sunshineenterprises.com.

Income verification documents can include: W2, Payroll Stub, Benefit Letter/Documentation, Unemployment Documentation, Proof of Disability Payments or Personal Affidavit.

If you are awarded financial assistance, the amount will be stated in your acceptance email.

If you selected NO above, your registration fee is $175, $250 or $325 and will be stated in your acceptance email.

If you are accepted into the Community Business Academy, you will be notified by email and are required to pay a $25 deposit (applied to the registration fee), to reserve your spot.

The balance of the registration fee must be paid before or on the first day of class.

If you are unable to make the complete registration fee payment, you are required to email a payment plan to recruitment@sunshineenterprises.com.

The registration fee must be completely paid by the 6th week of class.

SERVICE AGREEMENT

BY SUBMITTING YOUR APPLICATION, YOU ARE AGREEING TO THE FOLLOWING CONDITIONS

YOU CERTIFY THAT IF ACCEPTED INTO THE COMMUNITY BUSINESS ACADEMY:

You will be able to attend every class for the time slot chosen
You will arrive on time for each class
You are able to attend for the entire length of the class
You will complete and submit all take-home assignments
You will pay the Materials & Registration Fee, in full, on the first day of class*
You agree to participate in follow-up surveys conducted by Sunshine Enterprises for the next 5 years

DISCLAIMER
All Services and Documents are provided by Sunshine Enterprises, Inc. on an “as is” and “as available” basis without any express or implied warranties, representations, or guarantees of any kind, including, but not limited to, any of merchantability, fitness for a particular purpose, title or non-infringement, or arising from course of dealing, trade, or performance.

LIMITATION OF LIABILITY
Neither Sunshine Enterprises. nor its employees or shareholders (the “Sunshine Enterprises Parties”) shall be liable for any special, indirect, consequential, incidental, punitive or exemplary damages, even in the event of the foreseeability or advisement of such damages to or the negligence of the Sunshine Enterprises. Parties, or in the event of a failure of essential purpose and regardless of the theory of liability. In no event shall Sunshine Enterprises' liability to Client, even for direct damages, exceed $100. Client must bring any claim against the Sunshine Enterprises. Parties within one (1) year of the event giving rise to Client’s cause of action, and failure to do so nullifies the claim.

PAYMENT TERMS
I understand that I am receiving subsidized services from Sunshine Enterprises. and that if I accept admission into The Community Business Academy, I will pay a Materials & Registration fee of $175, $250 or $325, depending on my household income level. I will bring this Fee in full on the first day of class. Upon attending a minimum of one class of The Community Business Academy, this fee is non-refundable.

RELEASE OF INFORMATION
I give Sunshine Enterprises. permission to release my personal and business information to its funding sources. I also give Sunshine Enterprises. permission to use my personal and business information for program management and research purposes. I understand that the use of this information by Sunshine Enterprises will be limited to funding sources and research, unless otherwise required by law or to protect Sunshine Enterprises' business interests. I agree to release any claims I may have against Sunshine Enterprises and its employees arising out of or related to the release of personal and business information as provided in this authorization.
All of the application information is complete and accurate to the best of my knowledge. I understand that submitting this application does not guarantee acceptance into The Community Business Academy.

PHOTO PERMISSION
I give permission to Sunshine Enterprises and its subsidiaries, (collectively, “Sunshine Enterprises”), to use my likeness, alone or with others, by means of photograph, video or other medium chosen by Sunshine Enterprises for use in promotional materials or publicity. I understand my likeness may appear in internal publications, promotional audio/visual presentations, or external publicity, including, without limitation, use on Sunshine Enterprises' website or its periodic newsletters or annual report.
I agree that I shall have no right to (i) pre-approve Sunshine Enterprises' use of my likeness as described above, (ii) claim compensation for Sunshine Enterprises' use of my likeness as described above, and (iii) make any claims based on Sunshine Enterprises' use or discontinuance of use of my likeness as described above, including any claims for libel, defamation or invasion of privacy. I agree that Sunshine Enterprises shall own all images taken by it or its representatives.
I understand that if my image appears on Sunshine Enterprises' website, it may be viewed and downloaded by any computer. I waive any claims against Sunshine Enterprises and agree not to hold Sunshine Enterprises responsible for use of my image by a third party.
I have the right to contract in my own name. I have read this agreement and I fully understand it. This permission shall be binding on me and my heirs, legal representatives and assigns.
